Ninety percent of people have heard of a living will but only 29% of people have them. Advance directives are something we all should take the time to have in place for our own good and for our loved ones. National Healthcare Decisions Day (@NHDD) is an initiative encouraging people to express their wishes regarding healthcare and for providers and facilities to respect those wishes, whatever they may be. This week’s Friday Five gathers some motivation and tips to help you get started with your own planning and documentation.

Advance Care Planning Video from OpenLearn
Advance Care Planning (ACP) is the process of thinking about, documenting and sharing one’s wishes, beliefs, values and preferences for one’s future care. This process is often captured in a document called an advance care plan. The plan and the conversations around it can be used to help align and coordinate care around these preferences. This means that the care people receive towards the end of their life is more likely to be the kind of care they want. This video video comes from OpenLearn, a free learning platform, delivered by The Open University.
