FQHCs are recognized pioneers in understanding how social determinants can impact patient health, outcomes and overall system costs. As Medicare and other commercial payers continue to move toward population health and value based care, Craig provides some great insights on how organizations can use what we know about the impact of social determinants of health to improve patients’ immediate health, long term wellness and, accordingly, cost to the system.
